TSDA evaluation forms are provided as a resource for cardiothoracic surgery program directors, program coordinators, faculty, and residents. Evaluation forms can be used to meet Residency Review Committee (RRC) guidelines for evaluating residents, faculty, and the program as a whole. Programs are responsible for maintaining and filing all relevant documents and evaluations; TSDA does not maintain individual program files.

STS Thoracic Surgical Curriculum Templates

Template 1: Two-year curriculum: 34 weeks cardiovascular surgery followed by 42 weeks of thoracic surgery; may reverse for thoracic surgery for 42 weeks and cardiovascular surgery for 34 weeks; congenital disease for 12 weeks is separate and covered while on congenital service.

Template 2: Two-year curriculum: Cardiovascular surgery (34 weeks) alternating with thoracic surgery; must cover both basic and advanced; congenital disease for 12 weeks is separate and covered while on congenital service.

Template 3: Two-year curriculum: Cardiovascular surgery (34 weeks) alternating with thoracic surgery (42 weeks) alternating with congenital disease (12 weeks); must cover both basic and advanced levels.

Template 4: Three-year curriculum: Cardiovascular surgery (34 topics) alternating with thoracic surgery (42 topics) over 114 weeks; for basic and advanced; congenital disease (12 topics) over 18 weeks is separate and covered while on congenital service.

Template 5: Three-year curriculum: Cardiovascular surgery alternating with thoracic surgery alternating with congenital disease; 88 topics over 132 weeks; must cover basic and advanced levels.

Template 6: Integrated six-year curriculum: Based on Template 4 above with basic level for PGY 1-3 and advanced level for PGY 4-6 ; congenital disease (12 topics) over 18 weeks is separate and covered while on congenital service.

Template 7: Integrated six-year curriculum: Based on Template 5 above with basic level for PGY 1-4 and advanced level for PGY 4-6.
