It is obvious that with the introduction of the Bentall composite graft procedure in 1968, the operative results for these patients are outstanding. All major Marfan centers currently report operative mortality for this operation of less than 5%. Unfortunately though, Osler’s aphorism still pertains in 1999. Every year many Marfan patients go undiagnosed even when they present with Marfanoid physical features. Also a Marfan patient may have an aneurysm that is 6.0, 7.0, or 8.0 cm in diameter and have a normal appearing chest x-ray. We complete this discussion of surgery for Marfan aneurysm with several examples of missed diagnosis.
